In November 2022, Kelly’s world completely changed when she learned through our program that she had stage 4 kidney disease. As someone who struggles with extreme anxiety, this news was very unsettling for her.
Kelly’s registered nurse care manager, Heather Leep, quickly jumped in and helped Kelly develop a care plan. Heather detailed Kelly’s options step by step and helped her choose a nephrologist covered in her network who happened to align with the Interwell Provider Network to address her concerns. Heather was able to ease Kelly’s anxiety by outlining the options available to her and what to expect. Kelly felt like she was not alone in her kidney care journey and was relieved that our team was there for her. She was able to choose to safely start peritoneal dialysis and is currently preparing for a kidney transplant.
In the meantime, Heather also connected Kelly to a dietitian on the team. Working together, they were able to lower Kelly’s A1C from 13 to 6.1, preventing further damage to her kidneys. Kelly is optimistic and proud of the changes in her health and is confident in her team and the treatment plan. She knows exactly what to expect when she receives her kidney transplant and how to continue taking care of her health. Kelly no longer feels the anxiety she felt on the first day and attributes that success to her Interwell Health care team.
